How to Apply
To begin the admissions process, prospective residents or their representatives must complete an initial intake application. The application gathers important background information about the individual's mental health status, care needs, medication regimen, and support goals.
Once the application is reviewed, the admissions team will contact the applicant to schedule a pre-admission interview. This meeting may take place in person, virtually, or by phone depending on circumstances. The interview provides an opportunity for both parties to ensure McBride Supportive Living is a good match for the individual's needs and goals.
Admission decisions are based on a comprehensive eligibility review, a clinical and psychosocial assessment, and the current availability of appropriate residential accommodations. Priority is given to applicants who demonstrate readiness to live in a supportive environment and actively engage in personal growth activities.
Important Admission Notes
- All applicants must complete a medical clearance form verifying the absence of active communicable diseases.
- Proof of guardianship must be provided when applicable.
- All residents must comply with community rules and participate in basic activities of daily living (ADLs).
- Participation in psychiatric or therapeutic services outside the facility may be required as part of individual treatment planning.
McBride Supportive Living encourages early application submissions, as space within our supportive living program is limited.
